Qxd 12/22/08 32 11:31 Page 32 CHAPTER 2 space between the myocytes and the cells are less well organized; the nuclei are large and polyploidy is unusual. The diameter of the myocyte in the fetal sheep heart is only 5 –7 μm. The diameter of the myocyte increases by only a small amount during gestational development, so that the increase in muscle mass during prenatal growth is almost exclusively the result of increase in cell numbers, or hyperplasia. Following birth there is a rapid decrease in myocyte mitosis; a small increase in cell number occurs during the first few weeks after birth, but beyond this early neonatal period, essentially all myocardial growth is the result of increase in myocyte size, or hypertrophy.
Kluckow et al. have reviewed these observations. It is estimated that about 75% of SVC flow is derived from the brain. About one-third of all infants born at less than 30 weeks’ gestation had SVC flows below the estimated normal levels recorded within 24 hours of birth, predominantly in the postnatal period of 5–12 hours. In their studies there was a poor relationship between systemic arterial hypotension and the magnitude of SVC flow. There was, however, an excellent relationship between the observation of reduced SVC flow and the development of periventricular/intraventricular hemorrhage.
